Did You Receive Your Parent and (5th – 11th  Grade) Student Survey?

Parents and Students,

You should have received a we are asking you to complete.  The surveys serve two purposes: (1) ensuring students have access to supports needed this summer and (2) collecting information to assist in preparing to return to school this fall.

Student Survey The survey will be sent to students in grades 5-11. Students will be asked to verify their identity by entering their six-digit student ID the first screen of the survey. The survey includes questions about food and housing insecurity. If a student indicates that they need assistance in either of these areas, their contact information will be emailed to the staff at the DMPS Family Call Center the morning after they fill out the survey for follow-up.
